Below are some standard forms I use for my photography which can be downloaded or printed as required. I try to keep paperwork down to a minimum but there is always some.

Model Release
i)
This is my standard model release form that I use for all paid work and requires a signature on the date of the shoot. I also require to sight photographic identification of some form such as a drivers license or passport for any nude, partial nude or implied nude work. Please bring this I.D. to the shoot or we will not be able to continue.

This is my standard model release form that I use for "time for print/cd" work. I also require to sight photographic identification of some form such as a drivers license or passport for any nude, partial nude or implied nude work. Please bring this I.D. to the shoot or we will not be able to continue.

iii)
A PDF I have put together with a bit of information on Tulamben and the surrounding areas. It includes both macro and wide-angle photography as well as some information about the localities. I also outline the resort I stay at as well as some non-diving activities.

A PDF I have put together with a bit of information on some of the sites around Perth and southern Western Australia including the wrecks of HMAS Swan, HMAS Perth and the Lena. Not all the sites found here but just an outline of some of the locations I have frequented over the years.

ABOUT
Born in Adelaide, Australia I went to school locally and a subject that grabbed me at an early age was photography. I mostly had "hand me down" SLR cameras with a number of not so good lenses but I persisted and found a facination with black and white photography along with developing my own film. At an early age I developed an interest in underwater photography so at the age of 15 I built my own housing from perspex. In it I had a kodak instamatic camera that was very limited but, it worked.
